Request for Faculty Notification Letters
For Students Registered with Student Disability Services

If you are registered with SDS (i.e., submitted documentation and met with an SDS counselor) and classroom accommodations were finalized, you may use the on-line form below to request Faculty Notification Letters.

Letters to instructors listing the agreed upon accommodations you received in your last semester will be ready for you to pick up in 2 (two) business days from the date of your submission (pick up in 420 CCC). If you wish to modify your accommodations, you must meet with an SDS counselor to discuss your request. Please call 607-254-4545 to schedule an appointment.

NOTE: Letters granting classroom accommodations (including test accommodations) should be delivered to professors at least two weeks before accommodations are expected to begin (one week for Summer or Winter sessions), sooner for accommodations that need to begin earlier.

If you have not yet formally registered with SDS, please do not submit this form. Call 607-254-4545 to make an appointment to discuss your accommodation requests with an SDS counselor.
